Malnutrition in Slovak Hospitals

At the Healthcare Summit 2023 in December, a block titled "Correct Nutrition Also Heals Patients" caught our particular attention, focusing primarily on the nutritional status of patients in hospitals. We anticipated that this status would be far from optimal, but the numbers from a survey conducted by the Slovak Society for Parenteral and Enteral Nutrition (SSPEV), with support from the international educational grant of the Association of Clinical Nutrition Manufacturers (AVKV), took us by surprise.

The condition of malnutrition among patients in Slovak hospitals is indeed alarming. The survey, conducted in November and December 2021 through a questionnaire filled out by healthcare professionals, sampled 752 patients in departments of geriatrics, surgery, oncology, neurology, and internal medicine in the following hospitals: Poprad Hospital, Specialized Hospital St. Svorad Zobor, Svet Zdravia Banská Stiavnica, UN Bratislava - Kramáre, UN Martin, UNsP Milosrdný Bratia, and Rimavská Sobota General Hospital.

The results indicate that only about one-third of patients (35.9%) achieved a normal nutritional status. A staggering 40.6% of patients were identified as at risk of malnutrition, with 23.5% of patients demonstrably suffering from malnutrition. Malnutrition has proven to lead to higher morbidity, more infections, and an increased risk of complications, worsening recovery and wound healing.

The Slovak healthcare system has significant deficiencies in this regard, including the absence of a nutritional management system, outdated and inadequate legislation, lacking control mechanisms, the absence of nutritional screening upon admission or discharge from the hospital, the absence of nutritional therapists in hospitals, the lack of codes in the payment mechanism (DRG system), and a shortage of education for doctors in the field of nutritional care during medical studies.

In the future, it will be necessary to adopt health policies leading to improvement in this condition, requiring additional financial resources for coverage.
